DOT and CDL Exams

Department of Transportation medical exams are required for anyone who drives a commercial vehicle, to ensure that the drivers are fit physically, mentally and emotionally. You can learn more about medical examiner’s certificates here.

At Eureka Family Health & Wellness, your membership covers the cost of DOT/CDL medical exams*, making it easy and affordable for you to get on the road.

*Non-Member DOT & CDL Exam fee $120

FAA Flight Physical

Flight physicals are currently being offered of Class 2 and Class 3 pilots. If you are just beginning your pilot training and don’t currently have your initial pilot license, please visit the Flight Standards District Offices for instruction on your first certifying exam.

At Eureka Family Health & Wellness, your membership covers the cost of FAA Flight Physicals*, making it simply for you to get flying.

* May incur additional charges for any excessive time required for completion. Non-Member FAA Flight Physical fee $150.

Who needs a DOT medical exam?

You are a driver who operate in interstate commerce

You are a driver that operates a motor vehicle with a gross vehicle weight rating, gross combination weight rating, gross vehicle weight , or gross combination weight of 10,001 pounds or more

You are a driver that transports hazardous materials, in a quantity requiring placards

You are a driver that operates a motor vehicle designed or used to transport more than 15 passengers (including the driver)

You operate a motor vehicle designed or used to transport between 9 and 15 passengers, for direct compensation, beyond 75 air miles from the driver’s normal work-reporting location

Intrastate certification is regulated by the DMV of each state. All states have based their physical qualifications on some of the Federal requirements, but may grant waivers for certain medical conditions
